Supported Living

Footprints Care LTD purpose is to always make a real and lasting difference for everyone we support. We provide behavioural care services to improve people’s wellbeing. Our supported living services provide innovative and person-centred support enabling people with Autism. Learning Disability, Mental Health and other Complex Care and other Challenging Behaviours to live as independently as possible within our Supported Living Accommodation.

Temporary Accommodation

A safe and stable place to live is every person’s right. Footprints Care aspire to provide support. We provide accommodation for homeless people and other vulnerable people, we help with financial and housing advice. We also assist with general support, employment support, medical services and schooling. We give accommodation and support to the following vulnerable groups:

  • Mother and child temporary accommodation
  • Care leavers temporary accommodation
  • Asylum seekers temporary accommodation
  • Ex Veterans temporary accommodation
  • Homeless families temporary accommodation

Supported Accommodation

Through a network of high quality facilities, we offer combined up care pathways for the people we support. We provide programmes which integrate healthcare, therapy, specialist residential, supported community living and education services that are tailored according to individual needs.

Progressive Outcomes

Our integrated strength is that we can provide a seamless transition for the individual as they progress between higher and lower dependency services based on their individual needs, growth, wants, goals and aspirations. This unique and bespoke approach ensures that every person has the opportunity to achieve the best possible outcomes and quality of life..

The support provided is person-centred and specifically tailored to meet every unique need, reinforced by our values that every individual has the right to live the life they choose for themselves at all times. Because everyone is different, we focus on the individual and put them at the core of everything we do. We involve them at every stage of their care and support where appropriate. We believe that every individual should feel empowered to live the way they want; where they want; how they want and with whom they choose in a safe and comfortable environment.

Autism Care and Support

Our support for autistic people is designed around your needs, interests and preferences and we encourage feedback from you about the work we’re doing. Our goal is to support each autistic person to achieve the best.

Disability Support Learning

A learning disability is when an individual has a reduced ability to understand information. Individuals with a learning disability can also find it difficult to communicate and can affect their independence.

Huntington’s Disease Support

Huntington’s disease is a hereditary disorder of the central nervous system which usually develops in adulthood. This condition can affect movement, cognition and behaviour.

Prader – Willi Syndrome Support

Is a very rare genetic condition affecting approximately 2000¹ adults in the UK. It is caused by a defect of chromosome 15 and affects the brain in a number of ways. This has a significant impact on the mental, physical and behavioural development.

Progressive Behavioural Support

Through our Progressive Behaviour Support (PBS) services, we specialise in supporting adults with autism, Learning Disabilities, Mental Health and other conditions who sometimes display behaviours that other people may find challenging.

Transforming Care Services

Footprints Care provides residential settings, designed to support individuals with complex needs, as part of the Transforming Care Agenda. Our aim is to assist people moving from a hospital setting to successfully transition to living in their own.

We offer a wide range of specialist care to meet the social, physical and emotional needs of all people we support. This includes:

  • Innovative support programmes in nurturing and caring environments.
  • Supported living and community outreach transition services.
  • Residential care tailored to the needs of the individual.
  • Independent living in safe accommodation.
  • Dedicated and highly qualified staff teams.
  • Access to education and paid employment opportunities for adults aged 18 years and over.

Assesments

Footprints Care understands the complexities of each person’s own unique needs. We work with the individual from initial assessment to identify the most appropriate placement or support package, whilst supporting their future goals and aspirations. As the support and enablement needs of people can change, we also offer a flexible and responsive approach which is reviewed on a continual basis.
